In the event that your Mobile Caregiver+ App is not working or you are having issues with your phone, you must fill out a timesheet in order to get paid for your time and verify services were performed.
You and the client must sign to verify.
In case of a call-out, notify staff as soon as possible. Then fill out this sheet before the end of your assigned shift. Also use this sheet to schedule future missed days.
An important part of caregiving is observing & documenting changes you see in your client. Use this form to record and report your observations to the Avant office staff.
